Hello; I know captain's are humans as well and they will probably make at least one mistake in their career, but getting caught been drunk while on the bridge or been sleeping while you were supposed to be on the bridge are at least stupid things to do. Now about the ports, every port usually sends a warf pilot on board of inbound ships to take control over the vessel because he knows better those waters, how deep, where is the deepest point, how to approach the port and of course with cooperation with tugboat captain's help the ship to "park" on spot. But as it seems on those unfortunate moments we saw , everything went down the wrong way. Excluding the vessels who had mechanical or electrical problems and failed, the rest were mostly human errors who should not be repeated. Easiest way? To design smaller ships again. Sometimes people mess up, makes mistakes, and call problems. Usually the mistakes you make are pretty mundane; but if you're a captain of a ship small mistakes can escalate very quickly.
