Hello; Mostly the crew and Captain are at fault, because they failed to do something like check for malfunctions onboard, improper navigation, go slow in crowded port areas, misunderstand weather info or keep up with weather updates, or just neglect warnings from other vessels. Ship inspections are very important and should be done frequently. Nowadays there are floating metal debris which can cause damage, but are visible to radars and scanners on board a ship. There is also Satellite images and cameras. It just depends on what type of a vessel one is on and who is the Skipper or the Captain. The ship that was hit by a bridge was caused by the bridge operator being drunk and lowering the bridge too soon. Two ships were passing thru. The ship got so much damage it was scraped. It had more damage than it was worth.
